SPEAR – SIMPLIFIED PNEUMOTHORAX EMERGENCY AIR RELEASE – The SPEAR ® (Simplified Pneumothorax Emergency Air Release) Decompression Needle is the latest innovation in needle decompression technology. The versatile device was developed directly from science, user feedback, and known gaps in the treatment of tension pneumothorax. Every design feature has been carefully designed to improve user interaction and increase the likelihood of success when life is at stake. The user-friendly device allows for either lateral decompression or traditional anterior needle decompression.
The SPEAR device features a sharper needle tip that reduces the force required to insert the needle, as well as a spin-lock feature that helps prevent premature needle separation from the catheter. To confirm depth and success, the device features centimeter depth markings and a removable, one-way, low-pressure check valve that produces an audible hiss as air passes through it. The one-way valve can be removed from the needle and reattached to the catheter or terminated completely according to local protocols. After needle removal, the remaining catheter is flexible, kink-resistant, and has a soft tip to reduce the impact of potential tissue contact. Additionally, the catheter has 3 holes along its length to facilitate air flow.

The latest TCCC guidelines allow for a 10G needle in addition to the traditional 14G product. The larger lumen increases the flow area by more than two and a half times. The additional length of the SPEAR device (total catheter length: 3.75 inches) should allow the user to reach and maintain access to the pleural space in the vast majority of victims.
